Tobacco companies complain that the FDA is unwilling to approve new alternative products — including e-cigarettes — that might help adults quit smoking. EVP use increases risks of nicotine addiction, drug-seeking behavior, mood disorders and long-term risks of avoidable premature morbidities and mortality. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76].

The tobacco industry profits from destroying health and is using these newer products to get a seat at the policy-making table with governments to lobby against health policies. The tobacco industry funds and promotes false evidence to argue that these products reduce harm, while at the same time heavily promoting these products to children and non-smokers and continuing to sell billions of cigarettes. Most e-cigarettes have a battery, a heating element, and place to hold a liquid (such as a cartridge or pod). The e-liquid usually contains nicotine, flavorings, and other chemicals.

Cig-a-likes may also contain low levels of cadmium, a toxic metal also found in cigarette smoke that can cause breathing problems and disease.11 More research is needed on the health consequences of repeated exposure to these chemicals. There are also reports of lung illnesses and deaths related to inhalation of certain vaping oils into the lungs, which have no way to filter out toxic ingredients. Tried and tested interventions, such as brief advice from health professionals, national toll-free quit lines and mobile and digital cessation services are recommended. Where economically feasible, governments should also consider promoting nicotine replacement therapies and non-nicotine pharmacotherapies for cessation. And it bears repeating that most e-cigarettes contain nicotine, which has known health effects beyond addiction.

Almost half (49.1%) of young people don’t know what to do with used e-cigarette pods and disposable devices. Daily use of EVPs increased from 2% in 2015 to 7.2% in 2019, greater than three-and-one-half times increase. Although the percentage decreased to 5% in 2021, it was still more than a two-and-one-half increase since 2015. The potential public health impact of e-cigarettes is a subject of hot debate. Despite their popularity, little is known about their health effects. Some suggest that e-cigarettes likely confer lower risk compared to combustible tobacco cigarettes, because they do not expose users to toxicants produced through combustion. Results of the study, published online ahead of print in the peer-reviewed Ochsner Journal, show alarming statistically significant and clinically important increases of the daily use of EVPs in U.S. adolescents. No matter how it’s delivered, nicotine is harmful for youth and young adults. E-cigarettes typically contain nicotine as well as other chemicals that are known to damage health. The U.S. Preventive Services Task Force, a group of health experts that makes recommendations about preventive health care, has concluded that evidence is insufficient to recommend e-cigarettes for smoking cessation in adults, including pregnant women. Participants were first recruited in May 2021, and data analysis was conducted in December 2022. But it is not responsible for the harmful effects of smoking, and nicotine does not cause cancer. People have safely used nicotine replacement therapy (NRT) to stop smoking for many years. Nicotine replacement therapy is prescribed by doctors or is available from pharmacies. However, the portion primarily using non-banned, tobacco-flavored e-cigarettes also decreased (from 20.1% to 15.6%). All pre-ban to post-ban changes in flavor use were statistically significant in paired t-test. Compared with respondents who used e-cigarettes daily, respondents who used e-cigarettes weekly were more likely to decrease their use of menthol and non-TM flavors and were also more likely to quit e-cigarettes (12% versus 3.7%).

Notably, the reverse was not true—students who said they smoked cigarettes were no more likely to report use of e-cigarettes when asked approximately 6 months later. Electronic cigarettes, or e-cigarettes, are tobacco products that have been sold in the U.S. for about a decade. They include e-pens, e-pipes, e-hookah, and e-cigars, known collectively as ENDS—electronic nicotine delivery systems. They’re also sometimes called JUULs, “vapes” and “vape pens.” E-cigarettes are the most commonly used tobacco products among kids—and it’s become an epidemic.
