What Does Vaping Do to Your Lungs?

About one-quarter of U.S. youth and young adults have ever tried e-cigarettes. While vaping might help you quit smoking, it probably won’t help you quit nicotine altogether. Some damage to your lungs from vaping can heal or get better with medications. Over time, constant irritation to your lungs can lead to health problems (like asthma and COPD) that won’t go away. ENDS may be manufactured to look like conventional combusted cigarettes, cigars, or pipes. Larger devices, such as tank systems or mods, bear little or no resemblance to cigarettes.

To address these concerns, in early 2020 the FDA announced an order that would stop most flavored vape pods (also known as cartridges) from being sold. Then, in April 2020, the agency began reviewing all U.S. e-cigarette products to determine which could stay on the market and which should be removed. The decision for each is based on whether the products offer more public health benefits (helping smokers quit) rather than harm (hooking youth on nicotine). Most are noncombustible which includes a battery, a heating element, and a liquid compartment, usually containing addictive nicotine, that is added to the e-liquid or included in the device.

Currently e-cigarette use among middle and high school students increased alarmingly between 2017 and 2018. The legislation prohibits the sale and distribution of flavored vape products flum pebble clear, including menthol. Our study analyzed recent e-cigarette use patterns among US adults to monitor existing policies and guide the development of strategies to address potential health risks and improve public health. A recent national survey showed that about 10% of U.S. youth believe e-cigarettes cause no harm, 62% believe they cause little or some harm, and 28% believe they cause a lot of harm when they are used some days but not every day. E-cigarettes are considered tobacco products because most of them contain nicotine, which comes from tobacco. The FDA reported earlier this year that 1.3 million more high school students use e-cigarettes now than in 2017. Although the FDA began regulating e-cigarettes and other tobacco products in August 2016, there has been a significant spike in their use.

Some resemble other tobacco products, such as cigarettes, cigars, or pipes. You may hear them referred to as e-cigs, e-cigars, vapes, vape pens, mods, carts, tanks, dab pens, e-hookah, ENDS, or by their brand name, like JUUL or Puff Bar. The act of using an e-cigarette device is often called “vaping” or “hitting a vape”. E-cigarettes are the most commonly used tobacco product among youth. Vape flavors like mint and mango appeal to young people and mask the harshness of nicotine. The small, discreet size of vapes and e-cigarettes make them easy to conceal at school and home.

Virtually all e-liquids used in Juul and other e-cigarettes contain nicotine. When the user puffs on the e-cigarette’s mouthpiece, the battery-operated heating element activates. The heating element vaporizes the e-liquid stored in the cartridge and releases an aerosol or vapor, which the user inhales. E-cigarettes come in a wide variety of shapes, sizes and designs, but they all work in pretty much the same way.

E-cigarette vapour has been reported to contain up to 7×1011 free radicals per puff.10 Both nicotine-free and nicotine-containing condensate induced a significant increase in ROS release from our AMs, which may explain the induction of apoptosis in nicotine-free liquid. There was significantly greater ROS production in AMs treated with nicotine-containing condensate than in nicotine-free condensate, once again suggesting both nicotine dependent and independent mechanisms at work. The effects of ECVC with and without were ameliorated by NAC and PI3K inhibition. A Foundation-led study presented at the 2024 Society for Research on Nicotine and Tobacco annual meeting found that monthly e-cigarette sales in California declined by 41% after statewide restrictions on the sale of flavored tobacco products took effect. However, the report also noted that the tobacco industry continues to market products to offset these declines, including non-menthol cigarettes that contain other cooling chemicals.

The FDA has not authorized any flavored e-cigarette products and reports that it has denied marketing applications for millions of flavored products. E-cigarette companies have filed over 60 lawsuits challenging the FDA’s marketing denial orders. In addition, while the FDA denied marketing applications for all Juul products in June 2022, it subsequently put that decision on hold in the face of a lawsuit by Juul.

These products may have reusable parts, or they may be disposable and only used once before they are thrown away. Fortunately, there has been a substantial drop in cases since they peaked in August and September of 2020. What’s more, researchers have also identified vitamin E acetate, a chemical added to some THC-containing vaping products, as the main—but possibly not the only—cause of the illness. The outbreak of EVALI cases emerged against a backdrop of an ongoing vaping epidemic among youth.

The most recent evidence suggests that e-cigarettes may pose their own unique health harms and that comparison to cigarettes may not be the only relevant question for determining their impact on individual health. Indeed, the growing evidence of potential health risks related to e-cigarette use has led some researchers to question whether e-cigarettes are safer than combustible cigarettes. As smoking has declined dramatically in the U.S., vaping has gained tremendous popularity very quickly. A 2018 study reported 20% of high schoolers vaped at least once in the past 30 days. Nevertheless, there is no doubt that the deleterious effects of battery output voltage are not comparable to those exerted by CS extracts [30] (Figs. 1 and 2). With regards to toxicity, little is known about the effects of humectants when they are heated and chronically inhaled. Studies have indicated that PG can induce respiratory irritation and increase the probability of asthma development [55, 56], and both PG and glycerol from e-cigarettes might reach concentrations sufficiently high to potentially cause irritation of the airways [57]. Indeed, the latter study established that one e-cigarette puff results in a PG exposure of 430–603 mg/m3, which is higher than the levels reported to cause airway irritation (average 309 mg/m3) based on a human study [55]. The same study established that one e-cigarette puff results in a glycerol exposure of 348–495 mg/m3 [57], which is close to the levels reported to cause airway irritation in rats (662 mg/m3) [58].

In 2018, the FDA restricted the sale of flavored e-cigarettes to adult-only venues, with the exception of tobacco, menthol and mint flavors, which can be sold wherever traditional cigarettes are sold. The fourth-generation vaporizers can also be customized and come with different types of heating coils — some intended for vaporizing solids, not liquids. “Since most of the health concerns about e-cigarettes have focused on nicotine, there is still much we do not know about e-cigarettes. Pediatricians are particularly concerned about the threats to child and adolescent health posed by e-cigarettes, including heightened risk of nicotine-dependence and other health concerns related to vaping. The American Academy of Pediatrics has joined with other organizations to urge the U.S. Food & Drug Administration to take immediate action to protect children and teens from e-cigarettes.

Never smokers and former smokers were more likely to quit e-cigarettes. While there were no significant differences in quitting all e-cigarette use after each of the three state bans, e-cigarette users in New York were less likely to use banned flavors after the ban than those in New Jersey. The use of menthol flavor was also higher among smokers than former and never smokers. The percent of non-TM flavors had a reverse pattern, being lowest among daily smokers (46.7%), higher progressively along with those smoked weekly or less, former smokers, and highest among never smokers (79.1%). After the ban, the percentages of e-cigarette users who primarily used tobacco, menthol, and non-TM flavors decreased generally except the menthol share among never smokers did not change. After the ban freemax disposable vape, the percentage of those who quit using e-cigarettes was highest among never smokers (23.1%) and former smokers (13.5%) compared with 3.2% and 7.8% among those who smoked daily and weekly, respectively.

Assuming that a conventional cigarette contains 0.172–1.702 mg of nicotine [51], the daily nicotine dose administered to these animals corresponds to 40–400 cigarettes for a 70 kg-adult, which is a dose of an extremely heavy smoker. We would argue that further studies with chronic administration of low doses of nicotine are required to clearly evaluate its impact on carcinogenicity. The study showed that the e-liquids of certain cig-a-like brands contain high levels of nickel and chromium, which may come from the nichrome heating coils of the vaporizing device.

Unlike traditional cigarettes, e-cigarettes are generally battery-operated and use a heating element to heat e-liquid from a cartridge (usually refillable), releasing a chemical-filled aerosol. Since 2016, the FDA regulates the flavours used in the e-cigarette market and has recently published an enforcement policy on unauthorised flavours, including fruit and mint flavours, which are more appealing to young users [77]. However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5].

According to Blaha, there are three reasons e-cigarettes may be particularly enticing to young people. Second, e-cigarettes have a lower per-use cost than traditional cigarettes. Among youth, e-cigarettes, especially the disposable kind, are more popular than any traditional tobacco product.

Validated 7-day point prevalence abstinence rates were affected less, and effect sizes for all outcomes were close to those predicted, but the lower quit rates have reduced the statistical power for the primary outcome. Another key limitation of open-label trials is that participants’ expectations can affect outcomes. In smoking cessation studies, the results can be biased if participants randomized to what they perceive as an inferior option are less likely to use their treatment or are more likely to drop out of the trial.

Enter e-cigarettes, which were new, high-tech, and came with no proven health risks. There were no long-term studies yet, but common sense dictated that if you wanted to quit inhaling tobacco through smoking, the least you could do was switch to e-cigarettes. To date, the FDA has authorized only 23 tobacco-flavored e-cigarette products and devices. These are the only vape products legally marketed and sold in the U.S. We do know from my research that they can cause acute adverse events. Some people have reported developing mouth lesions from using them too much.

But this flavoring is accompanied by other chemicals comprising the aerosol itself. A study in the American Heart Association journal suggests that e-cigarette flavorings may damage blood vessels and the heart. Harmful substances, trace metals, and other toxins have been found in e-cigarettes. Yet, despite the fact more research is needed, there is enough evidence implying the immediate health risks in using them brings. The negative health consequences from long-term tobacco use and smoking either cigarettes or cigars took decades to prove. Altamore says the FDA has rejected millions of other vaping devices and liquids submitted by the vape industry, including Marco’s Vapor’s own brand Custom Clouds.

Thousands of illegal vaping products imported from abroad are being sold in New Jersey to teens despite a state ban enacted four years ago that tried to curb the highly addictive habit, a group of experts told state legislators Monday. Research and the EVALI outbreak suggest that vaping marijuana is dangerous in general and may actually be more dangerous than vaping nicotine products or even smoking cigarettes. Resources are available to schools and families to help them identify vape devices, how they’re used, and the safety and health risks they present.

Although evidence is lacking, e-cigarettes for inhalation of nicotine may be beneficial in reducing adverse health effects related to the use of combustible tobacco products. However, e-cigarettes are not FDA approved as a smoking cessation tool and the amount of nicotine and other substances a person inhales from each nicotine cartridge remains unclear. When FDA “deemed” e-cigarettes as part of its jurisdiction in 2016, it gave e-cigarettes that were currently on the market two years to prepare premarket applications (known as PMTAs). These applications are what FDA uses to to determine whether new tobacco products are “appropriate for the protection of public health” before they are allowed on the market.

As originally drafted, the bill would have imposed the first statewide tax on electronic cigarette products, as well as setting the minimum age for purchases to 21 from 19, the age currently in state law. However, the tax provisions were stripped out of the bill in the state House. The report was released as the Legislature was considering a bill intended to reduce youth use of electronic cigarettes. Nearly half – 46% — of Alaska high school students reported trying an electronic vapor product at least once, the report said. But use of electronic cigarettes rose from 18% in 2015 – the first year that metric was tracked — to 26% in 2019. The White House announced on April 26 that it would indefinitely delay the ban on menthol cigarette sales.

The ingredients, including nicotine levels, vary between brands and flavors, but many products are not thoroughly or accurately labeled. From introducing appealing flavors to offering college scholarships, manufacturers and sellers of e-cigarettes aggressively target young people. There are few federal restrictions on e-cigarette marketing, allowing companies to promote their products through traditional outlets — such as TV and radio — despite a ban in 1971 on cigarette advertising in both outlets to reduce cigarette marketing to children. E-cigarette companies also take advantage of other marketing outlets, including the internet, retail environments, and recreational venues and events. Nicotine is an addictive substance, but its level of addictiveness can vary substantially depending on its mode of delivery. Nicotine delivered by the combustion of tobacco is the most addictive form.12 The rise in popularity of e-cigarettes that can deliver levels of nicotine similar to combustible cigarettes is causing concern about the potential risk for addiction.

When it comes to the question of whether it’s safer to vape or smoke, it’s a matter of degrees of danger. The FDA hasn’t regulated vaping cartridges as tightly as other products. Ingredient lists don’t always disclose everything in the product, so you don’t always know what’s in vaping fluids.

We tried to minimize this risk by only including participants who were willing to use any of the study products. Expectations may be also less of a problem among smokers in China, where stop-smoking medications and EC are much less popular than among smokers in the West.8-12 Indeed, 92% of trial participants had no previous experience with any stop-smoking treatment. Regarding adherence, all study participants set their TQD and all initiated their treatment. Product use during the initial 3-month study period was also similar in all 3 study arms, and relatively high. These findings are reassuring in that they suggest that the study results were unlikely to be affected by expectations.

Finally, diacetyl, pentanedione, O-vanillin, maltol, coumarin, and CAD induced significant IL-8 secretion from MM6 and U937 monocytes [86]. It should be borne in mind, however, that the concentrations assayed were in the supra-physiological range and it is likely that, once inhaled, these concentrations are not reached in the airway space. Indeed, one of the limitations of the study was that human cells are not exposed to e-liquids per se, but rather to the aerosols where the concentrations are lower [86].

The “vapor” inhaled also can cause inflammations in the mouth, eventually leading to gum disease. Additionally, “vaping” has been proven to destroy the mitochondria used in wound healing. Over time, inhaling the particles present in the “vapor” can cause what has become the well-known “smoker’s cough.” Smoking is the leading cause of death and disease in the United States and according to the FDA, teen electronic cigarette use has skyrocketed by nearly 80 percent in the past year.

Jacob eventually tried vaping THC as well, but ultimately continued with flavored nicotine because of the flavors; cherry was one of his favorites. He said it tasted great and he could easily hide it from me, or any other adult, by simply passing it off as candy should someone ask about the odor. While many of the products that are sold at 723 Vapor will likely be banned in January, Florence hopes that the lawsuit will help with fixing some of the wording to be able to continue selling hemp vape products. On the rare occasions I partake, instead of filling my nostrils with the taste of a warm crème brûlée, my mouth tastes like a street gutter for an hour. I like the nicotine, but everything else about the experience is viscerally revolting.

Neither the air‐exposed rats nor those exposed to e-cigarette vapor using SS heating elements developed respiratory distress. In contrast, 80% of the rats exposed to e-cigarette vapor using NC heating units developed clinical acute respiratory distress when a 70‐W power setting was employed. Thus, suggesting that operating units at higher than recommended settings can cause adverse effects.

Upon entering the blood, nicotine stimulates the adrenal glands to release the hormone epinephrine (adrenaline). Epinephrine stimulates the central nervous system and increases blood pressure, breathing, and heart rate. As with most addictive substances, nicotine activates the brain’s reward circuits and also increases levels of a chemical messenger in the brain called dopamine, which reinforces rewarding behaviors. Pleasure caused by nicotine’s interaction with the reward circuit motivates some people to use nicotine again and again, despite risks to their health and well-being. Vapes, vaporizers, vape pens, hookah pens, electronic cigarettes (e-cigarettes or e-cigs), e-cigars, and e-pipes are some of the many tobacco product terms used to describe electronic nicotine delivery systems (ENDS).

Tobacco and vaping devices contain nicotine, an ingredient that can lead to addiction, which is why so many people who smoke or vape find it difficult to quit. Both tobacco and vaping devices contain other harmful chemicals; burning tobacco can create these chemicals and vaping devices turn chemicals and flavorings into mist that combines with synthetic nicotine. First, we did not collect detailed information on respondents’ use of other types of tobacco products both before and after the ban.

For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. The agency said it would ban all e-cigarette flavors except tobacco, mint and menthol in retail stores. Fruity or sweet e-cig flavors can now only be purchased through age-restricted stores or online merchants that can verify a buyer’s age. A 2018 report from Truth Initiative, an anti-smoking group, found that 15- to 17-year-olds were 16 times more likely to have used a Juul e-cig than older age groups.

Cig-a-likes may also contain low levels of cadmium, a toxic metal also found in cigarette smoke that can cause breathing problems and disease.11 More research is needed on the health consequences of repeated exposure to these chemicals. There are also reports of lung illnesses and deaths related to inhalation of certain vaping oils into the lungs, which have no way to filter out toxic ingredients. As the menthol ban hangs in limbo, people who cannot quit smoking those products altogether could be advised to shift to less harmful alternatives. These so-called modified risk tobacco products (MRTPs) are less dangerous than traditional cigarettes, according to the Food and Drug Administration (FDA). But they have generated limited sales, in part because they use ineffective advertising and must undergo an onerous authorization process.

Research indicates that e-cigarettes may emit harmful substances, which means there could be health risks to the user and others through secondhand exposure. They are sometimes called e-cigs, e-hookahs, mods, vape pens, vapes, tank systems, and electronic nicotine delivery systems (ENDS). The harmful effects of CS and their deleterious consequences are both well recognised and widely investigated. However, and based on the studies carried out so far, it seems that e-cigarette consumption is less toxic than tobacco smoking.

E-cigarettes are battery-powered devices that generate an inhalable aerosol that usually contains nicotine. Waste management and hazardous waste disposal plants are not currently equipped to handle e-cigarette waste and federal regulations still have not caught up to the need for guidance on disposal of these types of small electronics containing toxic waste. Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. E-cigarettes not only pose substantial health risks to youth and young adults, they pose a significant environmental threat (see the Truth Initiative fact sheet on Tobacco and the Environment).

You may have seen ads or stories on the internet that say e-cigarettes are not harmful, or are a good way to help smokers quit smoking. However, doctors and researchers still have a lot to learn about the health effects of e-cigarettes. While e-cigarettes may be less harmful than regular cigarettes, this does not mean that they are harmless. E-cigarettes, devices that typically deliver nicotine, flavorings, and other additives to users through an inhaled aerosol, are a rapidly emerging trend, and are especially popular among youth and young adults.

Additionally, the CDC reports nearly 38 percent of all high schoolers and even 13 percent of middle schoolers have tried “vaping” at least once, and those statistics likely are underreported. As shown in Table 1, the majority of the 1624 respondents were males (61.2%), young adults between 25 and 34 years old (56.9%), Whites (59.4%), and had at least a bachelor’s degree (66.8%). The majority of respondents had used e-cigarettes between two and five years (69.6%) and 79.8% had moderate or strong intentions to quit. The percent of respondents saying they used e-cigarettes because of the added flavors or for quitting smoking cigarettes were 54.4% and 36.6%, respectively.

Vaping and smoking cannabis products is included in the definition of smoking under the MCIAA. The FDA reports an alarming 900% rise in e-cigarette use among high school students from 2011 to 2015 and the number of high school students using them increased 78% in 2018 alone. Some individuals who smoke combustible tobacco products, such as cigarettes, cigars, and pipes, have been turning to electronic cigarettes (e-cigarettes) as an alternative to combustibles and as a smoking cessation tool. E-cigarettes are also commonly referred to as electronic nicotine devices, alternate nicotine devices, hookahs, vape pens, or e-cigs. Use of e-cigarettes and inhalation of their contents is known as vaping (American Cancer Society horizon tech vape coils, 2022). Moreover, while the basic technology behind e-cigarettes is consistent, there is enormous variability within the product category and there is no typical e-cigarette. The products include different ingredients, different hardware and deliver highly variable amounts of nicotine and potentially toxic chemicals, including heavy metals such as cadmium, lead, nickel, tin and copper.
