In 2017, FDA extended the deadline for completed applications to August 2022. When several public health groups, including Truth Initiative, sued FDA, a federal court ordered the agency to require applications be submitted by May 2020. The deadline changed again to September 9, 2020, following FDA and tobacco industry requests for more time due to the COVID-19 pandemic. The potential public health impact of e-cigarettes is a subject of hot debate. Despite inconclusiveness about their short- and long-term health effects, e-cigarettes remain largely unregulated and their popularity among youth and young adults continues to rapidly escalate. At the same time, serious illness and deaths have been linked to their use and recent research could not conclusively determine that they posed fewer health risks than combustible cigarettes.
The FDA has never approved an e-cigarette as a smoking cessation device. Studies into whether the devices help people quit smoking have yielded mixed results. When Juul first hit the market in 2015, its vape liquid contained much higher levels of nicotine than e-cigs that had already been on the market. According to the 2017 NH Youth Risk Behavior Survey, e-cigarette among youth is double that of combustible cigarettes and more than a third of 12th grade males report currently using e-cigarettes. During this time, the brain is constantly rewiring itself, which makes it far more vulnerable to nicotine addiction. This is part of why teens who vape are nearly three times more likely to begin smoking cigarettes. While secondhand smoke from cigarettes causes serious harm to others, there is no evidence so far that vaping is harmful to people around you and any risks are likely to be very low. E-cigarettes are considered tobacco products because most of them contain nicotine, which comes from tobacco. The FDA reported earlier this year that 1.3 million more high school students use e-cigarettes now than in 2017. Although the FDA began regulating e-cigarettes and other tobacco products in August 2016, there has been a significant spike in their use.
Using e-cigarettes, or “vaping,” are terms used synonymously to refer to the use of a wide variety of electronic, battery-operated devices that aerosolize, but do not burn, liquids to release nicotine and other substances. Marketing tactics targeting young people have contributed to the rapid increase in use.
For example, some Juul pods contain more nicotine than many other e-cigarettes. Research is mixed on whether vaping is safer than smoking, and e-cigs like Juul are so new that not a lot of research has been done. In November 2018, the FDA announced restrictions on the sale of flavored e-cigarette vape fluids. In September 2019, the Wall Street Journal reported that the FDA had opened a criminal investigation into Juul Labs to determine whether the company used deceptive advertising and targeted minors.
Despite their popularity, little is known about their health effects. Some suggest that e-cigarettes likely confer lower risk compared to combustible tobacco cigarettes, because they do not expose users to toxicants produced through combustion. Proponents of e-cigarette use also tout the potential benefits of e-cigarettes as devices that could help combustible tobacco cigarette smokers to quit and thereby reduce tobacco-related health risks. Others are concerned about the exposure to potentially toxic substances contained in e-cigarette emissions, especially in individuals who have never used tobacco products such as youth and young adults.
The liquid solution used in e-cigarettes, sometimes called vape juice or e-liquid, usually contains nicotine and other chemicals. E-cigarettes can be used to smoke or “vape” marijuana products, herbs, waxes and oils. Finally, the report discusses steps that e-cigarette companies took in 2021 to deter or prevent underage consumers from visiting their websites, signing up for mailing lists and loyalty programs, or buying e-cigarette products online. These steps include the use of online self-certification to verify users were at least 21 years old and following state laws requiring an adult signature upon delivery of e-cigarette products.
Those primarily using menthol before the ban were both less likely to quit and less likely to continue the same flavor after the ban than those primarily using non-TM flavors before the ban. Those primarily using non-flavored e-cigarettes before the ban were more likely to quit than those primarily used tobacco flavor before the ban. People who used e-cigarettes daily before the ban were less likely to quit and more likely to keep using non-TM flavors.

Like many states, New Jersey made it illegal to sell vaping products to anyone under 21 and then went a step further by banning flavored vape juice. Nicotine is the primary agent in regular cigarettes and e-cigarettes, and it is highly addictive. It causes you to crave a smoke and suffer withdrawal symptoms if you ignore the craving. It raises your blood pressure and spikes your adrenaline, which increases your heart rate and the likelihood of having a heart attack. Between 2011 and 2018, past-30-day e-cigarette use grew dramatically among middle school (grades 6-8) and high school (grades 9-12) students. Indeed, the latter study established that one e-cigarette puff results in a PG exposure of 430–603 mg/m3, which is higher than the levels reported to cause airway irritation (average 309 mg/m3) based on a human study [55]. The same study established that one e-cigarette puff results in a glycerol exposure of 348–495 mg/m3 [57], which is close to the levels reported to cause airway irritation in rats (662 mg/m3) [58].

However, the various flavors and flashy marketing campaigns attracted the attention of teenagers, public health experts say. Research on the link between vaping and COVID-19 outcomes is rapidly evolving and more research is needed to clarify the relationship. However, there is evidence that indicates that vaping exacerbates COVID-19 infection rates. The last five years have seen enormous spikes in youth e-cigarette use rates. However, additional follow up studies are required in COVID-19 pandemic to clarify the effect of e-cigarette use on lung and cardiovascular complications derived from SARS-CoV-2 infection. In this particular aspect, again the composition of the e-liquid varies significantly among different commercial brands [4, 35]. The most common and major components of e-liquids are PG or 1,2-propanediol, and glycerol or glycerine (propane-1,2,3-triol). Youth use of tobacco products—in any form, including electronic nicotine delivery systems (ENDS) like e-cigarettes—is unsafe. Such products contain nicotine, which is highly addictive and can harm the developing adolescent brain. Using nicotine in adolescence may also increase risk for future addiction to other drugs. Tobacco use continues to be the leading cause of preventable disease and death in the United States. There are no safe tobacco products; however, the health risks for tobacco products exist on a continuum, with combustible products such as cigarettes being the most harmful. Decades of research have documented that cigarette smoking harms nearly every organ of the body; cigarette smoke contains nearly 7,000 chemicals, approximately 70 of which cause cancer1.
The FDA has approved two drugs that are synthetic forms of cannabis for this purpose. They also can be used to treat anorexia in people with acquired immunodeficiency syndrome (AIDS). Some people prefer to vape marijuana due to the milder smell, ease of use, affordability, and dose consistency. Some studies suggest that in some ways (lower carbon monoxide risk, for example) it may be safer than smoking.
